Weight loss can slow after a few months at one dose. A slower week or month does not, by itself, call for a higher prescription.
Your clinician may look at your meals, activity, side effects, and scale trend first. Leave the syringe units alone until that’s all reviewed and taken into count.

It happens. Travel, a sick kid, a week that got away from you.

The right dose depends on whether you're a few hours late or a full week late, how many shots you've skipped, and what dose you're on right now.
How late are you?
Whatever the timing, never draw extra units to make up a missed dose.
If Your Dose Is Late
Some prescriptions include a window during which you can administer a late dose, something like "take within 3 days." If you see it, follow it and get back on schedule.
If there's no time window, message the care team or call the pharmacy before you inject.
Ask two things: When should I take this one, and when's the next one?
If You Missed an Entire Week
You may notice less appetite control as more time passes after your scheduled injection. When this happens, don’t make up for it by using an increased amount of medication.
Missed a dose altogether? Your medication instructions should tell you what to do next. If they don't, check with your healthcare provider.
If You Missed Two or More Doses
Contact your care team before restarting. Your clinician will likely require you to start on a lower dose or possibly complete a revised titration regimen to decrease the likelihood of adverse reactions upon returning to weight loss treatment.
When contacting the care team, provide:
- The date of your last injection
- Your prescribed dose in milligrams
- Your vial concentration
- The syringe units in your instructions
- The number of doses you missed
- Any symptoms you are experiencing
Those details give your care team what they need to review the timing and tell you how to resume your schedule. Until they respond, do not guess at a replacement dose or change the number of units on your own.
Does Semaglutide Dosing Change if My Vial Includes B12?
Your vial may list B12 along with semaglutide. For the calculator, use the concentration listed for semaglutide. B12 doesn’t change the conversion formula.
While our team hasn’t seen enough evidence that B12 helps with weight loss or prevents side effects, if your prescriber put it in your vial anyway, that's between the two of you.
Before You Draw: a Semaglutide Syringe Check
Pick up the vial and read the mg/mL off the label. Don't go by memory. Pharmacies switch strengths between shipments more often than you'd think, and the box looks the same either way.
- Concentration. Read it off the vial in your hand. A new shipment can be a different strength from the last one.
- Dose. Match your prescribed milligrams to that concentration in the chart or the calculator.
- Syringe. Confirm it's U-100 and marked in units. If it's marked in mL, use the mL figure instead.
- Scale. Check which mark matches your instructions. Ask your pharmacy if the result falls between markings or exceeds your syringe's capacity.
- Bubbles. Tap and clear them before injecting.
- Change. If today's unit count is different from last time and nobody told you to expect that, stop and call before injecting.
And please, if the markings are unclear in any way, take a minute to chat with your provider. The FDA has reported dosing errors from confusion between 5 and 50 units.
